Cornerstone Community Health Free Clinic 2015 Annual Report

  • Seventy-eight volunteers donated over 1700 hours in 2015; we held 52 clinic sessions:
    • 612 Overall visits
    • 289 Physician visits
    • 49 Nurse visits
    • 129 Physical Therapy visits
    • 145 Hygiene visits; >60 medications given or prescribed
  • Our patients were 49% Latino, 50% female, 24% uninsured, and 15% homeless; 42% report no primary care provider besides the Free Clinic, 25% report at least one disability, 57% receive government aid, and 81% are unemployed.  The average age of our patients is 42, with 7% of our patients below age 18 and 9% above age 65.
  • Thirteen percent of our patients report diabetes, 22% high blood pressure, and 7% high cholesterol.  47% of our patients smoke, 27% currently use alcohol, and 48% used drugs regularly in their lifetime.
  • We increased lifestyle counseling significantly over the year during our AAFP Foundation-Funded Lifestyle is Medicine grant.  We found that screening every patient at every visit was a first-step towards improving counseling in our clinic population.  We will present a poster on this information at the American College of Preventive Medicine Conference in Washington, DC in February 2016.
  • Patients increased daily fruit/veggie consumption from 2.3 servings daily to 3.2 servings daily over the year; patients with less produce consumption were more likely to be counseled, more likely to be older, more likely to be homeless, and more likely not to have primary care access.
